Cellophane Maculopathy

Cellophane Maculopathy or Macular Pucker is a pathology related to the central part of the retina, the macula. It is age-related and is characterised by the presence of a fibrotic membrane on the inner layer of the retina, which leads to a "wrinkling" of the macular surface.

This pathological mechanism is perceived by the patient with image distortion («metamorphopsia») and a gradual decrease in visual acuity.

Symptoms of Cellophane Maculopathy

In the initial stages, Cellophane Maculopathy manifests itself with symptoms that are not particularly pronounced. As the disease progresses, it leads to the appearance of a more or less large 'spot' (scotoma) that, in effect, prevents central vision. 

Worsening is never sudden and in many people the disease remains stable for a long time.

Causes of Cellophane Maculopathy

The causes of Cellophane Maculopathy are not yet known. Ageing of the vitreous body, with alteration of its structure (vitreous synaeresis), with the onset of posterior detachment of the vitreous itself, is in most cases the main cause of this phenomenon. 

However, there are other factors that can lead to its onset: 
 

  • Retinal detachment or tear
  • Ocular contusion trauma
  • Inflammatory eye diseases (uveitis)
  • Surgery to remove a cataract
  • Presence of metabolic diseases such as diabetes

Cellophane Maculopathy Diagnosis

Cellophane maculopathy is diagnosed during an eye examination, that is, by means of:
 

  • Examination of the ocular fundus
  • OCT
  • Fluoroangiography

Treatment of Cellophane Maculopathy

The treatment of Macular Pucker involves minimally invasive vitreo-retinal surgery. Vitrectomy is performed by making small accesses on the surface of the eye, through which microprobes are inserted to act on the vitreous body and the retina.

Treatment of Cellophane Maculopathy - Recovery Time

Recovery times vary depending on the severity (staging) of the disease. An absolute rest period of 15-20 days is certainly indicated.

Complete functional recovery can also be achieved at a distance: signs of functional recovery of the macular region subjected to peeling can be observed even a year after surgery.
