Minimally Invasive Vitreo-Retinal Surgery
Minimally invasive vitreo-retinal surgery is surgery performed with small-gauge operating instruments: from the traditional 20-23 Gauges to 25-27 Gauges, that is half- or one-third-calibre instruments.
Access routes for surgery (sclerotomies) are the transconjunctival incision (the conjunctiva is not opened) and sclerotomies often do not require a stitch at the end of the procedure.
The constructive reliability of small-gauge vitreous cutters and devices (vitreous forceps, scissors, laser probes, illumination probes) is maximum and completely superimposable on that of larger gauge. The small gauge of vitreous cutters requires high suction values, which has led manufacturers to increase the cutting performance of vitreous cutters, to the benefit of the reliability and safety of their intraoperative use.
The low invasiveness is determined by the fact that the reduced gauge of the accesses induces a lower inflammatory reaction, therefore less tissue involvement in the repair processes. There is certainly greater patient compliance, due to reduced healing times and faster functional recovery.

The pre- and postoperative examinations are those of vitreo-retinal surgery, that is, optical coherence tomography (OCT) or ultra-wide field imaging (OPTOS), which provide key information on the anatomical and functional status of the retina before and after surgery.
Recovery Time for Minimally Invasive Vitreo-Retinal Surgery
A few days of postoperative rest, accompanied by local antibiotic and anti-inflammatory therapy, may be sufficient for full functional recovery, as is already the case after cataract surgery.
